To address the condition, a procedure called Functional Endoscopic Sinus Surgery (FESS) was performed under general anesthesia. However, after the surgery, Mr. Agarwal's muscle tone and power did not recover adequately for adequate respiration, despite the reversal of anesthesia.
The anesthesia and ENT teams considered the possibility of cholinesterase deficiency, a rare condition that affects the elimination of anesthesia. Laboratory testing confirmed the diagnosis, with Mr. Agarwal's cholinesterase level at only 255, significantly below the normal range of 5900 to 12200.
To treat the deficiency, the patient received one unit of fresh whole blood, as blood from first-degree relatives could not be used due to the risk of the same enzyme deficiency. This treatment proved successful, and within 12 hours, Mr. Agarwal was weaned off the ventilator, extubated, and discharged the next day without any weakness or further support required.
The multidisciplinary team at Zydus Hospitals, including surgical, medical, anesthesia, critical care, and laboratory departments, worked together to promptly diagnose and treat this rare case. Mr. Agarwal's primary complaint of sinusitis with headaches has significantly improved, and he is doing well.
