In 2019, a young female presented at Zydus Hospitals with a large pituitary macroadenoma affecting her optic chiasma. After consultation, Dr. Dipak Patel, our Senior Neurosurgeon, referred her to Dr. Om Lakhani, our Endocrinologist. In coordination with the Laboratory department, we found the 'high-dose hook effect,' or the “prozone phenomenon," where extremely high levels of prolactin (PRL) can interfere with the assay and produce low readings. This was one of those cases which was prevented by doing undiluted and in 1:100 dilution before releasing the report. Instead of surgery, our experts pursued a medication-based approach to 'melt' the tumor. The patient underwent treatment with cabergoline and hormone replacement therapy. Remarkably, after just one month of follow-up, a repeat MRI revealed a 1/5th reduction in tumor volume and restored normal visual fields. Fast forward to 2023, when the same patient returned to Zydus Hospitals for a different reason - the birth of her child. She was on steroids and was advised a CS delivery. Her sonography just 24 hours before the surgery showed oligohydramnios, which resulted in having to do the surgery under General Anesthesia. Dr. Namita Shah, our Gynecologist & High-risk Pregnancy Obstetrician, successfully performed a cesarean section to bring her child into the world. A would-be-mother and her husband recently traveled to Zydus Hospitals from Bhuj seeking medical help. Mother and child were recommended to stay in the Critical Care unit for one-day post-operation under Dr. Pratibha's and her team's observation.
