In 2019, a young female presented at Zydus Hospitals with a large pituitary macroadenoma affecting her optic chiasma. After consultation, Dr. Dipak Patel, our Senior Neurosurgeon, referred her to Dr. Om Lakhani, our Endocrinologist. In coordination with the Laboratory department, we found the 'high-dose hook effect,' or the “prozone phenomenon," where extremely high levels of prolactin (PRL) can interfere with the assay and produce low readings. This was one of those cases which was prevented by doing undiluted and in 1:100 dilution before releasing the report. Instead of surgery, our experts pursued a medication-based approach to 'melt' the tumor. The patient underwent treatment with cabergoline and hormone replacement therapy. Remarkably, after just one month of follow-up, a repeat MRI revealed a 1/5th reduction in tumor volume and restored normal visual fields. Fast forward to 2023, when the same patient returned to Zydus Hospitals for a different reason - the birth of her child. She was on steroids and was advised a CS delivery. Her sonography just 24 hours before the surgery showed oligohydramnios, which resulted in having to do the surgery under General Anesthesia. Dr. Namita Shah, our Gynecologist & High-risk Pregnancy Obstetrician, successfully performed a cesarean section to bring her child into the world. A would-be-mother and her husband recently traveled to Zydus Hospitals from Bhuj seeking medical help. Two obstetricians in Bhuj had given them a grim prognosis, stating that there was a risk to the life of both the mother and the baby. Hearing about Dr. Namita Shah, our Gynecologist & High-risk Pregnancy Obstetrician, they promptly sought her expertise. The patient presented with severe thrombocytopenia at full term, experiencing early labor pains. In addition, she had a history of a previous cesarean section and abnormal liver function, identified through tests recommended by Dr. Namita. Despite receiving platelet transfusions, her platelet count remained around 20,000, posing a significant risk to her life. With help from Haematologist Dr. Dilip Srinivasan, who was involved due to low platelet count, and Gastroenterologist Dr. Hardik Kotecha, who was involved due to abnormal liver function test results, the combined efforts of the OT team and Anesthesia specialists, comprehensive care was provided. Dr. Namita performed an emergency cesarean section, and a healthy baby was safely delivered under the supervision of Dr. Anita Srinivasan, our Neonatologist. Mother and child were recommended to stay in the Critical Care unit for one-day post-operation under Dr. Pratibha's and her team's observation. Baby Shanvi was just over a year old when she fell ill with a fever and cough. Her parents assumed it was a viral illness and waited for her to recover. However, her condition worsened, and she developed viral pneumonia, which eventually progressed to a lung abscess. Worried about her lack of improvement, her parents rushed her to the ER at Zydus Hospitals, where she arrived in critical condition. Our skilled pediatric surgeon, Dr. Parthiv Shah, and Dr. Ankit Mehta, our Pediatrician & Critical Care Specialist, quickly assessed her condition and recommended right lower lobectomy surgery. The procedure was successful, and Shanvi was discharged after a recovery period. In addition, the parents, knowing that other families in similar situations could benefit from their experience, helped make an awareness video for pediatric surgery and the PICU department of the hospital. We wish Shanvi a healthy and happy future, and we are honored to have been a part of her journey to recovery.
